Waiver and Release

Liability Release and Parental Consent

In consideration of the acceptance of my application for the camp program, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages or which may hereafter occur to my child as a result of their participation in said camp. This release is intended to discharge in advance the Hobe Sound Nature Center, its administration, staff, volunteers, and agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that some camp activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assignees.

Consent of the Parent or Guardian

I give consent for my child to participate in the above camp(s), and I execute the above liability release on my child’s behalf.

Consent for Treatment

I hereby give my consent to have the above applicant treated by emergency medical personnel, a physician, or surgeon, in case of sudden illness or injury while participating in the above activity. It is understood that the Hobe Sound Nature Center will provide no medical insurance for such treatment, and that the cost thereof will be at my expense.

Refunds

By registering for this camp, I understand and agree that no refunds or proration will be given for any reason. When possible, we will do our best to accommodate date change requests.

I have read and understood the foregoing registration liability release and parental consent form, and agree to all of its terms and conditions.

The Hobe Sound Nature Center Turtle Walk Terms and Conditions

In consideration of the acceptance of my registration for the evening turtle walk program, I hereby waive, release, and discharge any and all claims for damages for personal injury or property damages of which may hereafter occur to me and/or any guests in my party, including minor children, as a result of their participation in said turtle walk program. This release is intended to discharge in advance the Hobe Sound Nature Center, its administration, staff, volunteers, and agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that turtle walk activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks on behalf of me and all guests in my party, including minor children. It is further understood and agreed that this waiver, release, and assumption of risk is to be binding on my heirs and assignees and/or the heirs and assignees of relevant guests in my party.

Consent of the Parent or Guardian

All minors must be accompanied by an adult. I give consent for my minor child/children to participate in the turtle walk program and will assume responsibility for consent for all minor children in my party to participate in the turtle walk program. As such, I execute the above liability release on behalf of all minor children in my party.

Consent for Treatment

I hereby give my consent for myself and all members of my party, including minor children, to be treated by emergency medical personnel, a physician, or surgeon, in case of sudden illness or injury while participating in the above activity. It is understood that the Hobe Sound Nature Center will provide no medical insurance for such treatment and that the cost thereof will be at my, or a relevant member of my party’s, expense.

The Hobe Sound Nature Center Homeschool Series Terms and Conditions

Waiver and Release

Liability Release and Parental/Guardian Consent

In consideration of the acceptance of my child’s participation in the Homeschool Series program, I hereby waive, release, and discharge any and all claims for damages for personal injury or property damage which may hereafter occur to my child as a result of participation in said program. This release is intended to discharge in advance the Hobe Sound Nature Center, its administration, staff, volunteers, and agents from liability, even though that liability may arise out of perceived negligence on the part of the persons mentioned above.

It is understood that participation in educational and outdoor activities may involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks on behalf of my child. It is further understood and agreed that this waiver, release, and assumption of risk shall be binding on my heirs and assigns.

Homeschool Group Supervision Requirement

I understand that children participating as part of a homeschool group must be accompanied by at least one adult parent, guardian, or designated chaperone at all times during the program. For homeschool groups larger than ten (10) children, a minimum of two (2) adult chaperones is required. Chaperones are responsible for supervising children and ensuring compliance with all program rules and staff instructions.

Consent of the Parent or Guardian

I give consent for my child to participate in the Homeschool Series program and execute the above liability release on my child’s behalf.

Consent for Treatment

I hereby give my consent for my child to be treated by emergency medical personnel, a physician, or surgeon in the event of sudden illness or injury while participating in the program. I understand that the Hobe Sound Nature Center does not provide medical insurance for such treatment and that any associated costs are my responsibility.
